When a person with chronic alcohol use stops eating and drinks heavily, their body runs out of sugar stores and starts breaking down fat for energy. This produces ketones that lower blood pH. At the same time, a lack of vitamin B1 blocks the normal use of pyruvate for energy, forcing cells to convert it into lactate instead. The body does not shut down because the liver and kidneys continue to clear these acids over time, and giving vitamin B1 fixes the blocked energy pathway. High lactate levels happen, but the body still recovers because the acidosis is caused by reversible metabolic blocks, not tissue death or organ failure.
