When cholesterol levels in the blood drop very low, fat inside unstable artery plaques gets removed by immune cells, making the plaques harder and less likely to burst. This only happens when the plaque has a lot of fat to begin with; if the plaque is already low in fat, removing more cholesterol from the blood does not change the plaque or prevent heart attacks.
