When the immune system mistakes the thyroid-stimulating hormone receptor for a threat, it makes antibodies that bind to it. Some of these antibodies overactivate the thyroid, causing it to produce too much hormone. Other antibodies bind to the same receptor in the tissue behind the eyes, triggering inflammation and fat buildup. Immune cells called macrophages move into the eye area, swelling the eyelids and pushing the eyeball forward.

Scientists gave mice a virus that made their bodies attack their thyroid, making it overactive. About half of those mice also developed puffy eyes and fat buildup behind the eyes, just like people with thyroid eye disease do — showing that an overactive thyroid might cause those eye problems.
