When a muscle is injured, stem cells normally multiply and turn into new muscle fibers. A substance called PGE2 helps this process. A different enzyme breaks down PGE2, limiting its effect. When a weight-loss drug is given, it reduces nutrients in the body, which slows down the stem cells' ability to become muscle fibers. Blocking the enzyme that breaks down PGE2 allows more PGE2 to build up. This extra PGE2 activates a signal inside the stem cells that turns on genes needed for them to multiply and form larger muscle fibers, fixing the damage without changing how much weight is lost.
