In obese individuals, fat cells around the organs release more fatty acids, which bind to a receptor on the fat cells called TLR4. This triggers a chain reaction inside the cell that turns on a gene switch called NF-kB, which directly increases the production of TNF-alpha mRNA. This happens much more in the fat around the organs than under the skin because the organ fat has more of this receptor and more fatty acids.
