High blood sugar in type 2 diabetes causes sugar molecules to stick to collagen in joint cartilage, making it stiff and brittle. This stiff cartilage pushes on nearby cells, triggering them to produce destructive enzymes and inflammatory signals. At the same time, the sticky sugar molecules bind to a receptor on the cells, which also turns on the same destructive enzymes and inflammation. Together, the stiffening and the receptor binding create a cycle that breaks down cartilage faster, which shows up as damage on X-rays and MRIs.
