When blood sugar stays too high for too long, it damages the lining of blood vessels, making them stiff and less able to respond when blood pressure is lowered. This damage prevents the vessels from relaxing properly, so lowering blood pressure further doesn't improve blood flow or reduce strain on the heart. But when blood sugar is kept at normal levels, the blood vessels stay flexible and can respond to lower pressure by relaxing, which protects the heart and blood vessels.
