When a child receives etomidate for anesthesia, it blocks a key enzyme in the adrenal glands that makes cortisol. This causes cortisol levels in the blood to drop and stay low for about a day. Since cortisol moves from the blood into saliva, the amount of cortisol in saliva also drops. This drop is clear and consistent, so measuring cortisol in saliva shows exactly how much the anesthesia affected the body's stress system.
