During intense exercise, abnormally high blood pressure forces the heart to pump against greater resistance, making the left ventricle work harder. This constant extra strain causes the heart muscle to thicken and grow larger. At the same time, the increased pressure inside the heart during relaxation makes it harder for blood to flow in, stretching the left atrium and causing it to enlarge. These changes happen even in very fit individuals if their blood pressure spikes too high during exercise.
