In fatty liver, excess fat breakdown produces acetyl-CoA, but the liver cannot convert it efficiently into beta-hydroxybutyrate. Instead, this acetyl-CoA gets burned in the energy-producing cycle, which increases the building blocks needed to make glucose. This shift blocks beta-hydroxybutyrate production but does not affect acetoacetate production.
