Thyroidectomy is a surgery in which the entire thyroid gland is removed.
After complete surgical removal of the thyroid gland, residual thyroid tissue in the chest can continue to produce thyroid hormone, causing blood tests to show normal hormone levels and misleading doctors into believing the thyroid was fully removed, if only standard neck imaging is used.
After complete removal of the thyroid gland without hormone replacement, normal levels of thyroid-stimulating hormone and thyroglobulin in the blood indicate that some thyroid tissue remains elsewhere in the body, such as behind the sternum, rather than the surgery being fully successful.
After total thyroidectomy, chest MRI and thyroid scintigraphy can identify residual thyroid tissue in the chest that neck ultrasound misses, and this tissue is the source of ongoing thyroid hormone production.
Retrosternal thyrothymic remnants are rare embryological structures that can remain after thyroid surgery and continue producing thyroid hormone even when the thyroid appears to have been fully removed.
When surgeons are aware of embryological thyroid variants like the thyrothymic tract, they are more likely to identify and remove ectopic thyroid tissue during thyroid surgery, resulting in more complete removal of thyroid tissue.
