Clomiphene blocks estrogen signals in the brain, which tricks the pituitary into releasing more hormones that tell the testes to make sperm. In some men, this boosts sperm production enough to bring sperm back from zero. In others, the same hormone surge overstimulates the testes, causing sperm production to shut down completely. Because men differ in how their testes respond to these signals, some gain sperm and others lose it, so the total number of men without sperm stays about the same.
